Tryouts for all 7th and 8th graders will start the week of March 26, 2012 All Sports Medical forms are due on or before Thursday, March 15, 2012

*** All forms are to be handed in person to the school nurse***
*** These forms must also be
cosigned by our District doctor prior to the start of the tryouts. Only complete forms will be sent to the District doctor.
Pay-to-Participate Sports Program
Students who try out for and make a sports team will be
required to pay a one-time only, $125 interscholastic sports activity
fee. This one-time fee would cover a student's participation in multiple
sports, if applicable. Please know that, per state regulation and board
policy, physicals and the advance submission of required sports forms
are still required in order for any student to try out for
interscholastic sports at Eisenhower. Please read the important information above about submitting sports forms correctly.
